Advice on Prescription

We are delivering the Advice on Prescription project in partnership with clusters of GP surgeries, known as Primary Care Networks.

The Advice on Prescription project aims to bring together primary care and the voluntary sector, creating an integrated service that meets the diverse needs of patients. Our goal is to enhance health, wellbeing and resilience while reducing deprivation.

We offer personalised 1-to-1 advice sessions over the phone. Whether it’s navigating benefits, housing or employment issues, our team is here to help. Additionally, we can facilitate referrals to our other services, helping with challenges like debt management and Help to Claim Universal Credit.

Patients can access our services through referral from their GP or Social Prescriber. Alternatively, they can book their own appointment by scanning a QR code or following a link displayed on posters within participating surgeries.

To qualify for the project, individuals must be registered with one of the listed GP surgeries (or be residents of Cotgrave village for the Cotgrave service).

 

Supporting Families

Through the Supporting Families project we provide support and advice to families with children. Our tailored services are designed to address a wide spectrum of needs, ensuring every family receives the assistance they require.

We offer personalised 1-to-1 advice sessions over the phone. Whether it’s navigating benefits, housing or employment issues, our team is here to help. Additionally, we can facilitate referrals to our other services, helping with challenges like debt management and Help to Claim Universal Credit.

In addition to individual support, we organise group sessions and events, including coffee mornings. These gatherings provide opportunities to schedule appointments and learn more about our comprehensive range of services.

We maintain close partnerships with local schools and community settings, ensuring our support is easily accessible to families, however our advice is always confidential.

To qualify for our project, individuals must be responsible for children under 18 and reside in Nottingham City, Rushcliffe, or Gedling.

Booking an appointment is straightforward. School staff and support workers can arrange appointments on behalf of families, or families can directly schedule appointments via our online booking diary.

Multiple Sclerosis (MS)

The MS Project is as a dedicated benefits advice service for individuals living with Multiple Sclerosis (MS) and their primary caregivers. We provide clients with comprehensive information, expert advice and professional guidance to ensure they access the complete range of benefits they are entitled to.

Our assistance extends across all aspects of the benefits process, from drafting letters, navigating complex application forms and gathering supporting documents.

Additionally, we can facilitate referrals to our other services or to other agencies, for help with challenges like debt management, housing support and social care assessments.
